The pack year calculator above computes a standard measure of cumulative cigarette exposure. One pack-year is twenty cigarettes a day for one year, so the calculation is cigarettes per day divided by twenty, multiplied by years, summed across every period at a different rate. That last part is what most calculators skip, and it matters: very few people smoke the same amount for their whole smoking history.

The page also states the published screening criteria, because that is usually why someone is looking the number up. It states them as what the guideline says, and nothing more. This page does not decide anything, does not reassure, and does not alarm. It reports a number.
What This Pack Year Calculator Does
It sums exposure across up to three periods, so a history of ten a day through your twenties, a pack a day through your thirties and forties, and five a day since can be entered as it actually happened rather than flattened into a single average. It reports the total, the total years, the implied average rate and an approximate total cigarette count.
It then compares the total against the three numbers in the published USPSTF criteria — age band, pack-year threshold and years since quitting — and shows which of the three the entered values fall inside. That comparison is a reading of published text, not a clinical judgement, and the page says so beside the result rather than in small print.
This is a different measure from the money tools on the site. How to Use It
- Break your history into periods. Use the rate you actually smoked in each stretch rather than a lifetime average, because averaging first and multiplying second loses information the sum keeps.
- Enter years as decimals where it helps. Eighteen months is 1.5 years. The measure is continuous, so there is no need to round to whole years.
- Zero out unused periods. If your history is one rate for one stretch, fill period one and leave the rest at zero.
- Enter age and years since quitting. These feed only the criteria comparison. Use zero years since quitting if you currently smoke.
- Take the number to a clinician, not a conclusion. Pack-years is an input to a clinical conversation. It is not a result, a risk score or a diagnosis.
The Formula, and a Worked Example
Pack-years equal cigarettes per day divided by twenty, multiplied by years smoked. Twenty is the number of cigarettes in a standard pack, which is what makes a "pack" a unit at all. For several periods, compute each separately and add them.
Take the default values. Period one: 10 a day for 8 years is 10 ÷ 20 × 8 = 4.0 pack-years. Period two: 20 a day for 15 years is 20 ÷ 20 × 15 = 15.0 pack-years. Period three: 5 a day for 4 years is 5 ÷ 20 × 4 = 1.0 pack-years. The total is 20.0 pack-years across 27 years of smoking, an average of about 14.8 cigarettes a day, and roughly 146,000 cigarettes in total.
Notice what averaging first would have done. The mean rate of 14.8 a day over 27 years gives 14.8 ÷ 20 × 27, which is 20.0 — the same answer, because the average was weighted by the years. Take an unweighted average of the three rates instead, (10 + 20 + 5) ÷ 3 = 11.7, and you get 15.8 pack-years, which is wrong by more than four. That is the mistake this tool's three-period layout exists to prevent.
What the Screening Guideline Says
The 2021 recommendation from the US Preventive Services Task Force, Lung Cancer: Screening, states: the USPSTF recommends annual screening for lung cancer with low-dose computed tomography in adults aged 50 to 80 years who have a 20 pack-year smoking history and currently smoke or have quit within the past 15 years. It carries a Grade B recommendation, and the guideline adds that screening should be discontinued once a person has not smoked for 15 years, or develops a health problem that substantially limits life expectancy or the ability or willingness to have curative lung surgery.
Those are the published criteria in the guideline's own terms. They describe a population for whom the Task Force found net benefit, which is a different statement from a decision about any individual. The National Cancer Institute's lung cancer screening summary sets out what low-dose CT screening involves, including its documented harms — false positives, incidental findings, follow-up procedures and radiation exposure — which are part of why the decision is a clinical one rather than a threshold one.
Guidelines also differ by country and change over time. The 2021 US criteria replaced earlier ones with a higher age floor and a higher pack-year threshold, and national programmes elsewhere use different entry rules and risk models. A number that falls one side of a threshold in one country may fall the other side in another.
Why Pack-Years Is a Crude Measure
It multiplies two numbers and throws away everything else. It cannot distinguish someone who smoked forty a day for ten years from someone who smoked ten a day for forty years, though the exposure patterns and the intervening biology differ. It says nothing about depth of inhalation, tar and nicotine content, filter type, or the age at which smoking started, all of which appear in the epidemiological literature.
It is also blind to everything other than cigarettes. Pipes, cigars, roll-your-own and waterpipe use have no agreed conversion into pack-years, and the measure was never defined for them. Passive smoke exposure, occupational exposures such as asbestos and silica, radon and family history are all separate risk factors that a pack-year figure does not touch.
And it depends entirely on recall. Almost everyone estimating a rate from twenty years ago is estimating, and rounding to the nearest pack is common. A history recalled to the nearest five cigarettes a day across a decade carries several pack-years of uncertainty. That is worth knowing when a number lands near a published threshold: near a boundary, treat the figure as uncertain rather than settled.
Reading a Number Near a Threshold
Thresholds in guidelines are population instruments. They are set where the modelled net benefit across a whole population crosses a line, and they are deliberately simple so that they can be applied at scale. They are not statements that something changes in a person between 19.8 and 20.1 pack-years.
The practical consequence is that a figure just under a published threshold is not a clean answer, and neither is a figure just over one. Both are numbers to bring to a clinician who can weigh the rest — other exposures, family history, symptoms, comorbidities, and what the screening pathway available to you actually involves. That conversation is the point at which eligibility gets decided, and it is not a conversation a calculator can have.
