Paragraph one: credential, setting and population

State the role, your licence and its status, and the setting and patient population you work in. A clinical manager is deciding whether you can be oriented onto their unit rather than trained from scratch, and that judgement is made in the first two lines.

Paragraph two: one clinical result you can stand behind

Choose an outcome you can evidence — a patient load carried, a documentation or compliance improvement, a quality measure your unit moved — and be careful to attribute team results as team results. Overstating a clinical outcome is the fastest way to lose credibility with a manager who knows how the work runs.

Paragraph three: why this employer

This is where most letters fail. "I admire your commitment to excellence" could be sent to anyone. Name something real — a product, a recent announcement, how the team works, a service line or a quality programme the organisation is known for — and connect it to your own experience. If you genuinely cannot find anything specific to say, that is worth noticing before you apply.

Home health agencies hire therapists who can work unsupervised and document correctly, in that order. Clinical managers ask about visits per day and drive radius because that is how caseloads are built, then about OASIS accuracy, since assessment errors affect agency payment and audit risk. They probe safety judgement in real houses with stairs, rugs and no equipment. Caregiver teaching ability matters because the family carries out most of the plan.

Mistakes that cost home health physical therapist candidates

• Sending a clinic resume that shows equipment-dependent treatment plans with no home context
• Not mentioning OASIS at all, which is the documentation skill agencies screen for hardest
• Skipping territory and visit volume, the two numbers a home health scheduler needs to place you

What this role needs on paper

A Doctor of Physical Therapy degree, a passing National Physical Therapy Examination result and state licensure are required. Home health agencies add their own OASIS documentation competency and typically require annual refresher training as the assessment tool is revised. A driving licence, insured vehicle and acceptable motor vehicle record are standard requirements, along with CPR and agency-specific orientation.
